Your miles per hour of charging are a bit optimistic. They assume either slow speed driving or warm weather. You would not get such numbers at 65 mph nor if driving in cold weather. To be conservative, I'd use 4 miles/hour for L1 charging, 12 miles/hour for L2 charging at 3.3 kW, and 21 miles/hour for L2 charging at 6.0 kW. If you know that you are going to be driving slowly or in warm weather you can nudge them upward a bit.
Yes, I concur with dgpcolorado's numbers, not rumpole's. You can't get rumpole's distances unless you are getting more than 5 m/kWh. A few hypermilers do that consistently, but most of us are closer to 4 m/kWh. Many new drivers get only 3 to 3.5 m/kWh.

Note that the socalled "6.6kW" charger is not twice as fast as the 3.3kW charger because they are talking about different things. The 3.3kW charger provides 3.3kW to the battery. The "6.6kW" charger only provides 6.0kW to the battery given the same input voltage. That's why dgpcolorado called it 6.0, not 6.6.
